Swim/Dive/Cross Country Clearance 

THIS CLEARANCE IS FOR SWIM/DIVE AND CROSS COUNTRY ONLY, THIS DOES NOT GUARANTEE A SEASON BUT IS PREPARATION TO START THE PROCESS

Date: Friday, January 22 and M – F January 25^th – 29^th

Time: 4:00PM – 5:00PM

Location: Eastlake High School parking lot under the solar panels next to the tennis courts. This will be a drive through process, please stay in your car and wear a mask.

What do I bring? 

1. SIGNED COVID Waiver
2. Physical papers completed properly with all signatures.
3. SIGNED three page Athletic Profile/Parental Consents.
