Athletic Clearance Process

Step 1 – Register your athlete by going to…

Step 2 – Complete the registration and print out and SIGN the  “Athlete information”

Step 3 – Complete and SIGN the “Sports/Co-Curricular Participation Screening Risk Assessment”

Step 4 – Complete and get Physical signed

Step 5 – Turn in the following to the ASB windows

• Athlete Information(3 page report)
• Risk Assessment
• Completed and signed physical

You must do this before the first day of try-outs/practice. Packets must be turned at least 48 hours before try outs.
