Weather Delays & Cancellations

Welcome to Eastlake High School Titans Athletics.  If you are interested in participating in athletics at Eastlake you can click on the link “Coaches Contact Information/Tryout Dates” to see what sports we offer throughout the year.  You will also see coaches' contact information and tryouts dates. 

To register for a sport, please follow these steps:

  1. Please use this SUHSD Physical Form and take it to your physician or clinic and ensure they stamp the form and check “cleared for athletics” with a date.  Physicals must be from the United States and only our SUHSD Physical Form.  The only other form we accept is a Kaiser Form (please also turn in the medical history/risk assessment SUHSD Physical Form).  Physicals are valid for one year, but must cover you for the entire sports season.
  2. Please click on the link “Click here for the steps to Athletic Clearance.”  Once you register for a sport you will need to print out the confirmation page from Home Campus with wet signatures from student and guardian.
    1. Do not forget to upload your medical insurance on Home Campus.
    2. Please take a picture of your physical front and back and upload it on Home Campus.  One side should be with the stamp from the doctor and the other side should be the medical history/risk assessment page.  Both sides should be signed by a guardian.
  3. Once you complete the registration on Home Campus and the Athletic Physical, please click the link “Important Dates & Clearance Schedule” and choose the correct season tab to turn in the home campus page signed and athletic physical to get cleared by Coach Hernandez in ASB.  Give us 48-72 hours to review and you will receive an email stating you are “cleared” or “denied” or “practice only.”
  4. If you get “denied,” it is ok.  There will be a note stating what you are missing.  It could be some of these reasons:
    1. You did not upload BOTH pages of your physical.  Remember to upload the stamped side AND the medical history/risk assessment page.
    2. You did not upload your health insurance card.
    3. Your physical may expire during the season and you will need to get a new one.
    4. You answered a question wrong.  Students may be ineligible if they are graduating before June, turn 19 before June 15th going into their senior year, went to school for more than 8 semesters, or registered in less than 5 credits.
  5. If you get “practice only” that means you went to a different high school and will need to see the Athletic Director to go over transfer paperwork.
